The following information and forms are necessary to complete intake procedures (please note that admissions for The CALM Center are an entirely different procedure and all forms will be completed at the site):

Please download, print and complete this ADULT INTAKE PACKET of forms.

Please download, print and complete the forms in one of these: CHILD INTAKE PACKET Youth 0 – 9  or CHILD INTAKE PACKET Youth 10 – 17.

Please download and complete these FEE AGREEMENT forms.

Please bring them with you to your intake apppointment or fax them to:

Counseling & Recovery Services is a nonprofit funded through a contract with the Oklahoma Department of Mental Health & Substance Abuse Services, Medicaid, Medicare, and through charitable gifts. We also accept most private insurance as payment for services.
