Counseling & Recovery Services of Oklahoma is committed to collecting, securing, maintaining, and providing quality client health information.

We also are dedicated to:

  • Providing information in an efficient manner, generally within 3-5 business days, and
  • Protecting the confidentiality and security of our client’s health information.

To request your medical records,  please download and complete the AUTHORIZATION FOR RELEASE OF CONFIDENTIAL INFORMATION form and mail, fax, or email it to:
