The following information and forms are necessary to complete intake procedures (please note that admissions for YES Tulsa and The CALM Center are an entirely different procedure and all forms will be completed at the site):

- ADULT INTAKE PACKET
- CHILD INTAKE PACKET Youth 0 – 9 or CHILD INTAKE PACKET Youth 10 – 17.
