Transitioning Teens to Adult

 

Hundreds of young people with behavioral health challenges leave high school without independent living skills to make it on their own. Transition Services offers support, guidance and advocacy for these young people, ages 16-24, with the goal of successful independence.

Many young people with emotional, behavioral health and substance abuse problems face potential loneliness, depression, unemployment, homelessness and crime at a greater rate than other young adults.

Transition Services helps these young adults prepare for life on their own by connecting them with community services and caring adults.

Bridges to the future

Transition Services helps young people explore their strengths and create a plan to achieve goals.

A support system of people and community resources builds bridges to a better future by putting plans into action with:

  • Career consultation, job placement and coaching
  • Education assessment, planning, enrollment and support
  • Social and independent living skills training
  • Subsidized housing
  • Physicians and medication services
