Medication Clinic Assistant 22-008

Using principles/practices of the Sanctuary Model, the Medical Review Nurse is designed to maintain the patient on a low, but effective, level of the least intrusive medications, encourage normalization and prevent hospitalization. Must be a licensed registered nurse (RN or LPN) or CNA or MA with 3+ years of experience. Must be a Medical Healthcare Provider under Medicaid standards. Follow up with clients on missed appointments and assist to notify them of upcoming appointments. Triage client phone calls while discussing concerns and providing limited advice per knowledge base. Assist with vitals and phlebotomy when needed. Review labs obtained and notify clients and provider of out of range numbers and schedule follow ups as needed. Discussing client concerns with the provider and treatment team to identify solutions. Keep the emergency supply kit up to date with all needed supplies to attend to a medical emergency. Provide education/consultation to agency consumers on medication issues as needed. Keep current on all OSHA requirements and apprise the Executive Director of changes that impact this agency. Perform a basic screening to determine whether COD might exist and be able to refer the client for a formal diagnostic assessment for co-occurring disorders. Manage a crisis involving a client, including a threat of suicide or harm to others. Refer a client to the appropriate mental health or substance abuse treatment program/facility when needed and follow up to ensure the client receives needed care. Recognize classes of psychotropic medications, their actions, and be able to educate clients.
