Care Team Lead (22-003) Under the directions of the Program Manager, Care Team Lead/ Facilitator (CL) will coordinate and oversee the care coordination activities of the Community Behavioral Health Clinic (CCBHC) core clinical team members. The CL will ensure compassionate, effective, and efficient delivery of high-quality clinical care for clients. Assists in complex problem solving and development of CCBCH core team staff. Monitor’s client flow and ensures the quick and efficient processing of clients during huddles. The CL ensures that all core team staff communicate with empathy and courtesy to clients. (EEO)
