Care Coordinator (HH/WRAP) (22-007): Benefits include: 3% annual merit increase potential with up to 4% annual bonus, 4% 403B employer contribution, Medical Plans, Dental Plan, Vision Plan, Life and AD&D Plans, Disability Plans, Pet Insurance Plan, and Generous vacation and sick time. The role of Care Coordinator requires a bachelor’s degree in the field of social or human services from a recognized university. Must have or be eligible for Case Management certification. Must be able to coordinate behavioral health referrals, follow up to ensure linkage to provide targeted support services (behavior management, daily living skills, social skills) linkage with community supports. Must have or be eligible for Case Management II certification.  Minimum of one year experience in a mental health field or related social service field. Must demonstrate strong communication and organizational skills. Strong attention to detail and must be able to keep up with multiple priorities. Must demonstrate computer proficiency.
